I checked your form and I notice the email recipient (di*****productmarketing@tribune.com) is in the bounce list. Here's the reason:
I have removed the email recipient to the bounce list. There shouldn't be any problems now in terms of receiving the notification.
Please try calling your email provider and have them whitelist our mail servers and domain names. Here's a guide: https://www.jotform.com/help/145-Whitelisting-JotMails-IP-Addresses
