Getting a form disabled error but it is enabled.

    Your account was autosuspended by our phishing detector, which is why your form is currently disabled. Please note that collecting login credentials (Username and password) is strictly prohibited in Jotform and it is considered a violation of our terms. I noticed that your form previously had a Username and Password field. This is why your account was autosuspended by our phishing detector.

    However, since you've already removed these prohibited fields and changed the form to name and address field instead, I have now removed the suspension on your account. You should be able to use your forms successfully now. Please avoid adding login fields to your form to avoid getting autosuspended again. Thank you.
