We've already contacted our email / LotusNotes team to see if the jotform.com email address is being blocked. It is not. Any advice on how to fix this issue? When I send a test message, I do not get them either.
All email addresses are XXXX@amwater.com

Please note that the e-mail address get notification for that form is not in bounce list. In addition, when I checked mail logs, submission notifications were sent successfully.
However, please note that there is only one recipient e-mail declared for that form.
Did you check "firstname.lastname@example.org" to see submission notification? Did you also check your spam folder?
If not, please kindly check both of them and return us if the issue persist.
I can also suggest you to whitelist our Ip address.
