Form contact information not being published in Constact Contact

I also did the integration step and my form is not showing up in Constant Contact as being a campaign or being published. 

I have checked your JotForm and see that the Constant Contact is correctly integration on your form. Could you try changing the campaign to a different one and see if that works? You may also try removing the integration and reintegrating it again to see if that makes any difference.
