Conditional logic deleted on my form

It looks like all of the conditional logic I had for my form has disappeared.  I checked the fields and none of them have any conditional logic attached to them any longer. 

For example - when you click YES to ARE YOU MARRIED, many field and options should appear.

Don't know how that happened.  Can you go back and reset all conditional logic to the way it was August 31?
