Adding fields dynamically

Can you possibly suggest a widget or feature that may be able to help us? We need to provide a link that can capture a person name, email address, and cell phone. Once they're added, I would like it to show the information, and allow them to enter additional (names, emails, phones) and possibly number them as they go along>
