When I cloned the form I changed the name to reflect that. When I send the link in a email it is correct. However if I post it to FaceBook the original name shows up. No matter what I do I cannot get it to show properly on Facebook.
I went to the FAQ & followed instructions to no avail.
This is the original:

If you post this to Facebook it will say West Side Class of 87 which was the original title but if you send it in a email the EC Central title will pop up which is what I want. Please help! In the meantime I will keep trying

I did noticed that there was an inserted html tag in the form https://www.jotform.us/form/62399527057163 Title
I suggest you change the title/name in Preferences/Advance settings also. This should fix the issue.
Let us know if it does not.

1SmoothRiderReplied on September 3, 2016 at 12:45 PMThat was the only way I could get the original title to stop showing up on facebook. That only happened when I try to post to facebook. It does not happen if I post to Google or send in email

The problem is that Facebook is scraping the old meta description. I was able to replicate the issue when I tried sharing your link in my Facebook account.
I fixed your meta description in Facebook by using the Facebook Sharing Debugger. I scraped the new information and now it is fixed.
Here's a screenshot when I tried sharing your form URL:
As you can see, there is a <title> tags on it because you put that tags in the Page Title. I am not sure why you did that but you can change it in the Preferences wizard again.
