I was wondering if there is any way to customize the map location widget.
I would like to show a base location with a certain radius shown (i.e 50kms)
Then I would like another location shown which will correlate to an address input within the form.

Unforutnately, you are constrained to the features and configurations available to the Map Location Widget. Such as the setting for the Longitude, Latitude, and the zoom. Aside from that, there is none. Aesthetically, you can include CSS on the outset of the view of the map, not on the inside parts.
For using a map as a to show a location based on the address input, there is a different widget for that called the address map locator widget. You can refer to this link for the said widget: https://widgets.jotform.com/widget/address_map_locator
