I also see the form https://form.jotform.com/62437004126951 blank when I test on my browser.
May I ask if you have just imported this form from PDF document to your JotForm? It is not possible anymore to import PDF form into your My Forms. You'll have to create the form using the Form Builder.
We will wait for your response.

Is your form from adobe form central? The migration period for adobe pdf forms had expired long ago. It used to be uploaded using the Import form tool here

Try importing your form using the Import Forms button on the upper right.

This tool https://www.jotform.com/fillable-pdf-form-creator/ converts your existing form/s in your My Forms page to a PDF form.
User guide: How-to-create-a-PDF-form
so you must have an existing form first, then you can convert it to fillable PDF form using this tool.
