Your account has been suspended due to violation of terms of use: https://www.jotform.com/help/8-Terms-of-Use

"Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."

Asking for login credentials is not allowed, therefor, you account will remain suspended.

I have checked and your account is autosuspended by our phishing system. Inside of your form you have field that is asking for keyword "كلمة السر هنا". Considering that is against our terms of use to have fields that collect login credential, such as password or key word, our system has suspended your account. I have removed mentioned field from your form and I have activated your account. You form is now working.

Please do not create forms that are against our terms of use or your account will be suspended again.

Hope this will help. Let us know if you need further assistance.
