Can I take a client intake form I have made up and paste it to one of these to put on my website for clients to fill out?

    I just created my website via Weebly for my new meditation practice. I do bilingual mediations. I have a Client Intake form with specific questions and information. If I use your service, which I really want to do as it will allow clients to fill out my forms online for convenience and expediency, can I copy and paste my form onto one of yours and use that?

    JotForm forms can quite easily be added to Weebly pages.  Here is our guide on how to do so:

    Thank you. I appreciate the links. I was actually asking something else. The Client Intake forms I've seen on your website, don't include specific questions that I need to ask clients. Am I able to modify your forms or do you offer blank forms on which I can create what I want with the questions I want to ask?  Thanks again.
