Getting "502 Bad Gateway" error and entry duplication

Please help. My form doesn't seem to work at all right now. This is a form we've been using since August 10 and today it is returning a "502 Bat Gateway" error.

In addition, folks have a choice to make a payment online now or at the door of the event. When they choose to pay at the door, the entry gets duplicated every time 2 seconds after the first one. This has been a problem on other forms too.

I just checked the website, the form is displaying correctly.  Please let us know should you need  further assistance.
