Star Ratings disappear on iPhone and iPad.

When filling my form on a desktop all the star ratings are preserved and submit successfully. However on both iPad and iPhone as I enter a rating the rating in the previous question disappears and when I go back the rating I entered before is lost. I have made the form responsive but it makes no difference. 

Does anyone one know how to fix this?

I checked your form on an iOS emulator and I was able to see the issue you were referring to. When I click on the next star rating, the rating on the previous rating field disappears.

I did further tests on my own test form and I discovered that the Hover Text property is affecting the behavior of the Star Rating on iOS. If the Hover Text contains some text, then the issue occurs. But if I remove the text on the Hover Text area, then I am able to rate the Star Rating fields successfully.

To resolve the issue on your form, I would suggest to delete the text inside the Hover Text area of all your star rating fields.
