It was working before but now it isnt. Is there a limit to how many times it can be submitted? Do i need to pay or put the jotform logo on there? Please help me.

That form is on a Guest Account and have reached the submission limit. I would suggest creating a Free Account, which increases the limit a 100 per month. However, if you need more than that, please see http://www.jotform.com/pricing/
As soon as you create your free account, your form will be merged to that account. Make sure to use the same email address.
