    I believe you can use our QR Code Reader widget to achieve your requirement. Simply add this widget to your form and you can now read QR codes with your form. Here's the link to the widget: http://widgets.jotform.com/widget/qr_code_reader.

    Hi there, unfortunately it doesn't work on my android device / samsung phone as it says I need adobe flash 11.7.

    Unfortunately there is no other QR reader widget available in jotform aside from the current one. Here's a link to all our QR related widgets: http://widgets.jotform.com/search/qr

    The QR reader widget requires Adobe Flash to work, so you will need to install Adobe Flash to your device or use a Flash enabled browser to use the widget. You can follow this guide on How to install Adobe Flash on an Android smartphone or tablet: Get Flash on Android.
