Embedded form is not responsive in Adobe Muse

Hi, I'm having trouble with a form that fails to resize (just disappears instead) as the webpage becomes smaller. The page was created using Adobe Muse. The "make this form responsive" option was selected, as outlined in the instructions, but the problem persists. I've also tried a "mobile widget" as described in one of previous responses, all to no avail. The page in question is located at http://www.stomatolog.com.au          (third tab from the left).

However, the form is not responsive when you embedded it on your webpage. Please try to re-embed the form using the Iframe method. Here's a guide: Getting-the-form-iFrame-code
