How can I have a field with tree input boxes instead of adding three multiple fields?

    I have a customer who has requested that a field include three text boxes next to each other for a certain question where a person fills out information about the past three pay periods. He wants it to look like this:

    Any ideas on how I can do this? Any widgets that might make this happen?

    You can use our Fixed Format Widget for this: https://widgets.jotform.com/widget/fixed_format.

    In the widget configuration, you can use underscore to define the input boxes and you can use any separator your want.
