All my forms was disable.

    all my forms was disable as well

    Your account was autosuspended due to violation of Jotform's terms. This is why your forms are currently disabled. Please note that collecting login credentials (username and password) is strictly prohibited in Jotform. 

    To remove the suspension on your account, please delete the following fields on your "Vamos começar!" form that violates the terms.

    Once you have deleted these fields, let us know on this thread and we shall remove the suspension on your account accordingly. Regards.
