Problems Integrating with Salesforce

    Hello! I have a form that I am integrating with Salesforce. Everything works fine until I add the last two fields (which are check box fields). If I add either one - the entire form won't be received by Salesforce. If I remove the fields - then everything populates to Salesforce just fine. Is there anything on this side that is wrong with those fields? Do I need to change the names of the fields on the Salesforce side? Any help is appreciated! Thank you!

    I have tested the integration and it seems to be working on my end, please before to continue make sure that you're not submitting duplicated entries, if you're doing it then they simply will not be added to the integration:

    I have also set up a cloned form of yours and I can see it works:

    I'm sending the field to a custom check box field in my SalesForce account, here is the record generated from my testing submission:

    My custom field is a check box as well on the SalesForce account, which field type are you using on your end? 

    Please try removing and adding the integration once again, if it still does not work, please let us know, we will be glad to further test.
