This Form İs Disabled Hatası

Formumda bu hatayla karşılaştım. Kendi sitemde yetkili alımı için form yapmak istedim. Bunu nasıl çözebilirim.

I have checked your account and it has been disabled, this is because on this form https://form.jotformeu.com/62526199240356 you're asking for a username, here is a screenshot of the field:
