Right now I can only choose one radio button out of the list.
I tried working around by using a checklist but it looks very amateurish.
Any help you can provide is greatly appreciated.

Unfortunately, the purpose of the radio button is to choose only one item from the group of items available. The use of the checkbox on the other hand allows you to choose more than one.
If you find this amateurish there is a way how to make your checkbox appear more stylish.
So first, Simply edit your form and go to Design. A window should appear
Once the wizard appears (the window), click the advanced designer button at the bottom
Next, on the Advanced Designer Wizard, select the radio/checkbox items you would like to modify. Then click on the image that appears on the right part (as seen on the screenshot)
Next, choose your style
Once done, click save on the upper-left part of your browser
Note: that this styling applies to checkboxes and radio buttons. So either way, you can choose your preferred style of input.
