Embedded form not showing properly in Dreamweaver

The contact form shows up on Dreamweaver CC, but it is not centered.  How do I change the code to center it?  Also, it's supposed to have sky and clouds, but it doesn't.  It just has an outline.

The form itself is aligned in the center. However, if you embed it on a webpage, the alignment of the form will depend on the location where you embedded it.

Please provide the URL of the webpage where the form is embedded so that we can investigate about the alignment and the background image you're referring to. We cannot troubleshoot the issue if the website is not yet uploaded online.
