It is saying my form is disabled

How do I enable my form?

I checked your account, it is marked as 'autosuspended'.  Which is done by our anti-phishing system that detects suspicious forms and if they have some of the phishing attributes. I checked your form and I see that it has words like: username,password, which is forbidden by our Terms of Use:

I deleted those fields from your form. And activated your account, but please avoid putting this kind of fields on your form to avoid to be suspended again.
