I'm not getting subscribe confirmation email from Mailchimp.

I tested integration with Mailchimp and subscribers are going through.
Here is the form that I submitted:

Yes, I'm still having the problem. I do not see your test email in my Mailchimp list or in my API calls list.
When I enter an email, I do not get the confirmation subscription email from Mailchimp or my autoresponder emails.
Your screenshot does not match my form. My form only has first name and email address.

Are you referring to that email, that you're not receiving it?
I checked your integration and didn't find anything wrong there.
It could be the setting in your Mailchimp
