Survey form not collecting all answered questions, what is wrong?

when the survey form is completed and submitted the results are not all there. it is random on which question or answer is not showing in the results. all row are required to be completed

I opened your form but I wasn't able to replicate the problem. The error prompted me when I leave the field blank.

I cloned your form and check all the Matrix table. I can see that it is set to require every row to be filled up. Here's a screen capture:

Is this form embedded on a webpage? If it is, please try to re-embed the form using the Iframe method to prevent script conflicts between the form and the web page. Here's a guide: Getting-the-form-iFrame-code.

no it is not embedded. I'm just setting it up & testing it. it is not giving accurate results. are you saying I should not require every row be filled? but I need each question to be answered ?????

It is okay if it is set to Require every row to be filled. However, it seems that it is not working properly on your end. May I know what browser are you using? Can you please test the form in other browser just to check if it will work or not.

If the issue persists, please try selecting the Require an answer in every cell in the Matrix tables.
