My jot form doesnt seem to be enabled

I try to preview it and it says disabled. In the preferences it is enabled.

I see that your account was automatically suspended and the cause of it is because of the word "Credit Card" on the header of your form.

I have reactivated your account since it's a false positive (since you're using  Stripe to receive payments). But I would recommend to not use the word on your title. This is because it's causing your form to be flagged and your account to be auto-suspended.
