I think this is related to the fact that your account is currently suspended and it happened because you're asking for Credit Card details in your form.
Here is a screenshot of the guilty fields:
Please do note that asking for CC info goes against our terms, here is the link to them so you can read and know how to avoid being suspended: http://www.jotform.com/terms/
Also, in order to re-activate your account I had to remove those fields.
Do note also that it is a must to use our payment integrations if you want to collect CC details, here is the link to our current apps: https://apps.jotform.com/category/payment_processing

After activating your account I tested the PDF fillable form feature and I was able to download it:
