I am looking for something like this

    http://patriotconstableservice.com/app/index.html#/federalsubpoena/form

Is it possible to build this with jotform and the completed form sent back as a PDF?

    Thanks

    Hello Mark,

Yes, you can create such a form in JotForm and download a PDF version of the form submissions. You can likewise customize the layout of your PDF submissions.

Here is a guide to walk you through on how to customize your PDF form submissions: https://www.jotform.com/help/384-How-to-Customize-PDF-Submissions-Report

In any case, should you encounter any trouble in relation to building your form, please let us know. We'll be more than happy to assist.

    Thanks.

Yes, you can customize the form to look like the one in the link you have shared us, but looking at the layout it seems like you will need to add some CSS code to achieve that layout, here is a guide that will help you with injecting the code to your form: How-to-Inject-Custom-CSS-Codes

You may create your form in the Form Builder and then you may let us know so we can help you customizing your form.

Regarding to your question about the PDF report, I have moved it to another thread it can be found here: https://www.jotform.com/answers/931852

We will assist you as soon as possible on that thread.
