I am looking for something like this

Is it possible to build this with jotform and the completed form sent back as a PDF?

Yes, you can customize the form to look like the one in the link you have shared us, but looking at the layout it seems like you will need to add some CSS code to achieve that layout, here is a guide that will help you with injecting the code to your form: How-to-Inject-Custom-CSS-Codes
You may create your form in the Form Builder and then you may let us know so we can help you customizing your form.
