Sharing the form in Facebook does not show the correct title

The word "Elections" is nowhere in the form or the title.  I can't find it in the source code.  I need the link box that shows up on Facebook to have the correct title or people won't click on the form.  Can you please help me fix it so the link title shows as "Volunteer Application"?

I was able to replicate the issue. It seems that Facebook is not capturing the correct meta information. I used the Facebook Debugger tool to scrape the form meta data. This should be fixed now. Here's a screenshot:
