Why embedded form disappears on mobile devices?

on my site i have a jotform, ive added the mobile responsive widget but the form only shows up at the largest breakpoint. after the form gets to about 1200px it dissapears and once it disapears it doesnt come back as you widded the screen without a refresh. http://gbhdesigns.com/jennifer/contact.html

Could you please provide us more details about the mobile device you used to test your form? 

I have tested your form in a real mobile device with Android 5, I used Google  Chrome and this is my result: 

I have also tested your form in an iPhone 6 emulator and I can see the form loads fine as well. 

I can see you are using the iFrame code, could you please try re-embedding your form?

If there are any changes made to your form then they should reflect once the form is re-embedded, also if possible, please  try loading your webpage in another mobile device.

Yes, we do suggest to embed the form using the iFrame code since it helps to avoid any conflict between the form's code and the existing code in your website.
