I know I have been able to do this in the past, but cannot remember how now. I have 33 submissions, but sometimes it is only showing me 10 or 11. I do not want to print separate registrations, but get them all on one or two pages, in list form. I only need the fields described in the form, not an id number for each, for example. Can you help?
fields: name (first & last) phone number, email, church (optional)

Please take a look on this guide, there you should find the method that you used to download all your submissions in a report: How-to-Download-Form-Submissions-as-Excel-CSV-PDF
I have tested with the Excel file and I can see it downloads the correct number of submissions, you have currently 33 submissions in your form and they are complete in the report that I downloaded:
Regarding to hide fields in the report, you can do it as well, if you're generating an Excel report, for example, then this guide will help you: How-to-Create-an-Excel-Report
No matter the report that you're generating you're able to decide which fields will appear in it, here is a detailed guide about reports, please take a look: How-To-Create-Form-Reports-in-JotForm
