After I sent the first email test - it killed my email account.

Can you please explain further what you meant by your email account being killed? Do you mean you are unable to receive email notifications from your form? I checked your email address and it doesn't seem to be in the bounce list so you should receive email notifications successfully.
I checked your notification settings and it appears that there is no Email recipient in the notification.
This may be why you are not receiving any notifications from the form. Please setup your notification correctly to receive notifications successfully. Regards.

I re-enter it again.
Also, I was on the phone with my internet people for 3 hours trying to
get me email to work again - after I sent the first test - and we ended
up and had to kill my email and re-install it. I did get this message,
but the last message I received yesterday was the email that send the
code to verify my email.

You have to follow the process explained in this guide
In there notice that you can go to Recipients, and add the email there.
