Fillable PDF form: PDF form can not be created, working wheel is just spinning

The form I am trying to create a PDF of is named "SMSP Parish-wide Registration 2016". It's a lengthy form - I'm simply looking for a PDF to internally share the form with a colleague who will be managing responses. I don't need to accept responses or publish via PDF, just looking for all of the fields to print in an organized and concise fashion.

Are you trying to create Fillable PDF form of the form you provided? I have tried to create Fillable PDF form on your account and I was able to replicate mentioned issue. 

I am not sure what is causing this issue, but it is possible that your form can not be converted to Fillable PDF form because it is to long. 

I will escalate this issue to our developers and we will inform you via this thread if this issue can be resolved. 

Also if I understood you correctly, you want to print your form and use it as paper form. You can achieve that by adding Print Button to your form and printing empty form. 

Print Button on Forms
