Secondly how this is working for some but not others I can't figure out. When it can be filled out by other employees it will not send me a notification even though I have my email address in the notifications box.

Regarding the submission email not coming through, I checked and it ended in our Bounce list, the reason is below:Reason(s) : smtp; 554-mail.methodistfamily.org
I took necessary actions to remove it from our bounce list so you should be able to receive submissions now.
Regarding that issue please check this guide on how to preven it and also whitelisting of our domains would help as well.
