How can I delete this account?

Please delete my personal account associate with the email:  lhturner2009@ualr.edu or tell me how I can do it.  I couldn't find anything on your website.  I created it as a first step to assist our office, but have no need for it.

I apologize for the confusion.  We are overwhelmed with JotForm requests from professors, and three students took the wrong test today.  One of them took the wrong test TWICE even after the professor corrected us.  What a mess on our part.  Again, I apologize.  Thank you so much for your help.
