Is it possible to create login form registration?

i need a login/ website registration form big enough to block information behind it to pop up when someone goes to my website.  the customer should have choice to login if they have login account or register request account.  I should get that request and customer should only get an email confirming that their request is being reviewed.  

i have distribution business and customers need to be able to request account via website, so the form will have fields to put information or simply login.  if they login, screen moves away and they can see website, if they can only register, after they submit website and login screen goes away, but info gets sent to me.

Any templates for this or am i going to have to make this from scratch?

Unfortunately, it is not allowed to create Login forms by our Terms of Use. You can create your own login screen and then point it to our form.
