Blocking credit card number

A form we created is blocking the method of payment.  We have used this before and never had this happen.  Can you tell me how we can fix this?

Jotform became PCI complaint recently. Capture credit card critical information on plain text fields is a violation to our terms of use and it goes against to PCI compliance.  That is why we had to scan our database and replace the numbers with hash symbols.
