The Voice Recorder widget uses Flash to gain access to the end-user's microphone.
iOS devices don't natively support Flash Players on browsers so it will never work there to begin with. For MACs (desktops/laptops), it will highly depend if the browser being used has a flash plugin installed.
If you're using Safari on MAC, check out this help page https://helpx.adobe.com/flash-player/kb/enabling-flash-player-safari.html on how to enable Flash. But preferably, you should use Chrome.
This is evident when testing with Windows or browsers that have flash plugins. The Voice Recorder widget works properly on those. Sadly, this is beyond our control so if you really need to have this work on iOS, you'd have to wait for these devices/browsers to natively support Flash.

Use FastField Forms their forms use audio players and video players that work on EVERY platform.
