I did all the steps that were mentioned in the JotForm FAQ & have not received any email notifications.
Your immediate attention to this matter will be greatly appreciated.

I tested your form using my own email address and it works fine. I was able to receive the notification as well as the autoresponder.
May we kindly ask you to check your spam folder maybe, or try to login to your webmail and see if Jotform mails are marked as spam. If it is, please add it on the safe sender list.
I've also checked the Mail logs for email@example.com and says it was sent successfully./var/log/jotform/amazonSES.log:[1;36m20-04-2012 02:30:57[1;37m 82652513259 AmazonSES Sent email to firstname.lastname@example.org - MessageID:00000136ce724614-afa7af1b-0384-42e7-b0b6-a382315003e6-000000/62b73184-8ab2-11e1-a24c-6166f1605c91
