How do you correctly use the conditions, specifically the show/hide feature?

I am trying to use the show/hide option on some of my forms.  I have double checked my conditions and they appear right, but when I preview my form, it doesn't work properly.  Basically, when you answer the question that should "hide" a chunk of items, it just leaves the screen blank (it doesn't go to the next set of questions that apply to that question).

Are you referring to the conditions set for the field "Which type of assessment did you conduct?"? As I check the conditions set for this field, I notice that you are also showing/hiding the Page breaks which is not allowing the form to go to the next page. Please try deselecting the Page breaks fields from the conditions so that it should be working properly. Also, you may select all the fields in the same action of Show Multiple and it is not required to set different actions in the same condition.
