Two of the recipients in the form are showing failed logs. Your primary email address is not listed in our bounce list so this is probably because of your spam filter not accepting emails from Jotform, especially the PDF Attachment is enabled in your form.
I would suggest doing the following:
1. On your Outlook settings, add email@example.com to your safe sender or contact list.
2. Create a new notifier for your outlook email instead of adding it as one of the multiple recipients in the form.
