A couple customers have complained that their request was never addressed. They provide a snapshot of auto response to their request but it doesn't show up on our end.Page URL:

If I understand you correctly, you are having an issue receiving the notifications, is that correct?
The email recipient in your notification and your email address are not in the bounce list. I did a test submission and according to the mail logs, it was sent successfully. Here's a screenshot:
Please do check your spam folders or junk mails. The test submission also shows up in your account.
