Redirecting form users to different pages based on their selections

Wow -- your forms are great!

But, I have a question concerning my form at:

When I created a Condition using your template, it didn't provide an option to allow me to go to my web page when I press the Submit button.

Let me explain my Condition that I want to create:

If the user selects the Yes button, then, when he selects the Submit button, it will send him to my web page with the free ebook.

If the user selects the No button, then, it will send him to a different page or put up a message.

Can you help?
