It's just two lines -- name and email address. It was working fine in August but nothing but blanks in September. How do I tell if it's JotForms or WordPress that's dysfunctional? And how do I fix it?Page URL:

Meaning it can be submitted even though there is no values inputted. I did a test submission and it is showing as you can see below:
