I checked your account and confirmed the following:

2. 4:51PM You changed your registered email from louisetran@gmail.com to ltran@pdgmobility.com
And that's the reason why you never received it on ltran@pdgmobility.com The test submission you did was performed while your account email was still louisetran@gmail.com
To test this, open your form once again https://jotform.com/62595955057267 and do a test submission. You should now receive this on ltran@pdgmobility.com
