Need help - form looks good on page where I created and saved but loses middle section on preview and website.

I am trying to create a form that allows for personalizing either 4 or 5 lines with a limited number of characters. On the form creation (see screenshot) it looks perfect. On preview, this section of the form is missing. please help

They are hidden in your form because of the following incomplete condition (conditions with missing field). I have deleted this condition in your form. They should appear in your form now.
