How can I get my form to allow me to be able to select photo/Video from my phone?

Hi

I have created a form for my website and thats all well, but I have a Mobile App and have linked it to the webpage where my form is. It shows up, but i would like to be able to select the upload button and upload file/Photo/video from my phone?

I have tested your form and when I clicked on the "Upload File" button I got a menu to choose how the file will be uploaded, please see the screenshot below: 

It seem like you have embedded your form in a mobile app, please do note that this may be the reason why the upload button does not show the options, I would suggest you to redirect to the web version of your form instead, so the form opens in the web browser and it is submitted. 

However, please share us the link to your app so we can download and test on our end.
