Is there a way to completely customize the css/html using a code from codepen.io (for example : http://codepen.io/daliannyvieira/pen/xOygEW) ?
I tried to copy paste the html/css but it seems there is conflict with css from jotform. Is there a way to empty all css of jotform ?

Since the field classes are different, it would not be possible to directly apply that CSS to your form. The styles that were not for classes specific to the form could be copied directly.
However, anything that they used a specified class for would need to be updated for the specific JotForm fields.
