IT Event management Forms

Hi, we are struggling to provide events and IT notifications to the End Users and Business, to you have any ideas, or templates to help drive or develop forms for > IT System Outages, Resolutions of Outages, Scheduled Outages, Maintenance Outages etc.... I would like to have drop downs to enforce consistency.
