The text fields disappear once the form has been submitted. How to I keep the text once submitted so that once submitted, it is a full contract?

I created a contract where after each text field, I require initials.  But once the form is submitted, I only get the initials and not the text boxes.  Is there a way to fix that?

After the form submission, you will get the value entered by the user in text boxes, and in other fields. So can you please confirm, which value you are unable to receive on Form submissions.

I am also able to see few text-boxes are not mandatory to fill, so might be possible, user has not entered the value in those text-boxes.

If there is any specific text field, for which you are unable to receive the value of that, please let us know the name of that specific text field.

I am receiving their answers in the submissions, but I would also like to
be able to print out all of the text that I have created, once the form is
filled out.
It is for contract purposes, so the need for all info on final print out is
important.

You can actually print the submission together with the texts, headers, questions and answers. Here's a guide:

1. In My Forms page, select the form and then click the "Submissions" button.

2. In the Submissions page, navigate to the submission you want to print and then click the "Edit" button.

3. Now you'll see your form with the data entered by the user. Scroll down and click the "Print" button. The default browser printer wizard will appear.

Also, you can enable the PDF attachment in the email alerts so that once a new submissions comes in, you'll receive an email with a PDF attachment. The PDF contains the questions and answers.
